How Eczema Changes with Age
Eczema does not look the same on everyone. Its appearance and location often change as a child grows:
• In Babies (Infants): Eczema commonly appears as red, dry patches on the cheeks, face, trunk, and the outer sides of the elbows and knees.
• In Older Children and Teenagers: The rash frequently shifts to the creases of the skin, such as the inner elbows and behind the knees. Over time, frequent scratching can cause the skin in these areas to become thick, leathery, and deeply lined—a process called lichenification.
• Skin Tone Differences: On darker skin tones, eczema may look less red and more brown, purple, or grey. It may also appear as small bumps rather than flat patches.
Daily Skin Care: The Foundation of Control The most effective way to prevent eczema flare-ups is to build a strong, protective skin barrier:
• Moisturise Regularly: Apply a fragrance-free moisturiser or emollient daily. This traps moisture in the skin and protects it from irritants.
• Choose Gentle Cleansers: Use fragrance-free, non-soap body washes. Avoid harsh soaps or cleansers that strip the skin of its natural oils.
• Watch the Laundry: Wash clothes with additive-free detergents and avoid heavily scented fabric softeners.
• Wear Cotton: Whenever possible, wear soft, breathable cotton clothing next to the skin to prevent overheating and irritation. Managing Eczema Flare-Ups When the skin becomes red, itchy, or inflamed, it is known as a flare-up. Daily moisturisers alone are usually not enough to treat an active flare.
Medical treatments may be recommended to calm the inflammation:
1. Topical Treatments (Creams and Ointments)
• Targeted Calming Care: Doctors often prescribe mild topical options for delicate areas like the face , or stronger, mid-potency formulations for thicker skin on the arms and legs. These are typically applied twice daily during a flare until the skin clears.
• Steroid-Free Options: For sensitive areas like the face where long-term management is needed, non-steroidal options may be recommended to avoid skin thinning.
• Ointments vs. Creams: Ointments are often preferred because they form a better protective seal on the skin and enhance absorption. They also rarely contain the preservatives found in creams that can cause a stinging or burning sensation when applied to raw skin.
2. Spotting Complications (Infections) Eczema can sometimes become infected by bacteria because the broken skin barrier allows germs to enter. If you notice the skin is oozing, weeping, or forming yellowish crusts, it may require a short course of oral targeted antibiotics prescribed by a doctor.
3. Post-Eczema Light Spots (Pityriasis Alba) As eczema heals, it can leave behind faint, poorly defined light patches on the skin, especially on the face. This is called pityriasis alba. It is a temporary loss of pigment due to past inflammation and is not the same as vitiligo. These spots become more visible after sun exposure when normal skin darkens, and it can take several months for the skin color to return to normal. The Emotional and Family Impact Eczema is more than just a skin condition it can significantly impact a child's and family's quality of life:
• Sleep Disturbances: Severe itching often wakes children up at night, leading to daytime tiredness, irritability, and difficulty concentrating at school.
• Social and Emotional Wellness: Children may feel self-conscious or embarrassed about their skin lesions, which can affect their confidence, friendships, and participation in activities.
• Family Fatigue: Caring for a child with eczema requires significant time and can impact parents' sleep, peace of mind, and daily functioning. If your child's eczema is affecting their sleep, mood, or daily activities, please consult a medical professional. A customized management plan can help heal current flares and significantly reduce future ones.
